**WILL TRAVEL TO MASONICARE LOCATIONS AS NEEDED**
The Home Care Services Supervisor is a front-facing professional well-versed in Masonicare’s home care business line. The Supervisor has strong presence throughout Masonicare’s numerous residential and skilled nursing locations, and liaises with clients and the organization’s different business lines to meet individual client care needs.
Essential Duties and Responsibilities:
1.Executes tasks focused on quality care, relationship management, and operational efficiency. Serves as primary liaison between MAH and its clients, as well as between the Masonicare continuum and both current and potential clients as it relates to home care services.
2.Creates, reviews and updates individualized care plans in collaboration with clients, families, and healthcare professionals.
3.Serves as expert resource to caregivers and support staff. Timely addresses issues and concerns and escalates to Executive Director appropriately. Conducts routine and ad hoc site visits to uphold client satisfaction and quality care standards.
4.Carries out intake activities and tracking of referrals, and conducts necessary follow-up.
5.Conducts home care-related consultations and assessments to ensure safety and client satisfaction.
6.Trains and mentors care staff on company policies, care standards, and best practices.
7.Maintains accurate and timely documentation related to client care, visits, and communications.
8.Attends community and continuum events as invited/requested to embed Masonicare at Home in the communities we serve.
9.May assist with recruitment, interviewing, and selection of care staff.
10.Demonstrates strong oral and written communication skills as well as the ability to work independently and manage time.
11.Communicates effectively and tactfully with staff and clients, recognizing their age, cultural diversity, needs, abilities and physical condition.
12.Must be willing and able to travel throughout CT for majority of the workweek.
13.Performs other duties as deemed necessary by the Executive Director.
Minimum Qualifications:
Education:Bachelor’s degree in a health-related field strongly preferred. Will consider relevant experience in this field in lieu of degree.
Experience:Private duty homecare experience preferred with a strong knowledge of local markets and the geriatric field.
