Intake Specialist

Job Location: Wallingford Center, Connecticut

Full Time - Day Shift

9:00am - 5:30pm


*This role will be a combination of on-site in Wallingford, CT and remote*


Accepts and processes referrals, completes full data entry of referral, obtains and verifies insurance information and requests initial authorization for Home Health and Hospice admissions.



Essential Duties and Responsibilities:


  1. Accepts and processes appropriate referrals to the agency per agency policy in collaboration with Intake Nurses and coordinates all intake information in the management of referrals for service. 

  1. Assures coordination of all required services to clients and families by agency staff and community resources. 

  1. Assists with non-clinical support including, data entry, filing, answering phones, calling referral sources, and ordering supplies.

  1. Enters referral information into the agency health record including listed payers.

  1. Participates in the orientation of new staff members.

  1. Initiates collection of pertinent information related to face-to-face documentation requirements.

  1. Collaborates with Intake Nurse for final acceptance of complex referrals. 

  1. Maintains databases and pertinent information relative to the needs of the Intake Department.

  1. Works on special projects as needed.

  1. Communicates effectively and tactfully with multiple referral sources, maintaining a customer service focus with outside agencies as well as branch personnel and colleagues.

  1. Will report to work on time, use time off appropriately, and complete annual education & medical requirements. Works weekends and holidays on a rotating basis as determined by department needs.

  1. Participates in Weekend and Holiday rotation as needed and required.

  1. Perform other duties as required.




MINIMUM QUALIFICATIONS:

  1. High school Diploma or equivalency
  2. Experience with computer data entry, word processing, and creation of reports.
  3. Maintains current drivers’ license and auto insurance.
  4. Possess reliable transportation (Exception: Employees who utilize public transportation—Supervisory approval required).
  5. Previous experience with medical terminology or employment in a medical setting preferred.
