**ON-CALL ROTATION**



Essential Duties and Responsibilities:

  1. Develops and implements quality improvement processes throughout Masonicare Health Centers ensuring a standardized and unified approach across all the departments.
  1. Identifies and facilitates education needs regarding the key quality initiatives/targets/goals (based on federal, state and best practice guidelines) for Masonicare Health Centers.
  1. Tracks, analyzes, and collects data for the purpose of demonstrating the efficacy of our improvement processes as well as projecting a strong reputation to our patients, residents, potential partners and regulatory/best practice organizations.
  1. Develops the customer satisfaction process and relates findings to the QI process.
  1. Leads the development of yearly and long-term quality management plans, including required and specialty audits based on service delivery trends and/or general agency/industry trends. 
  1. Performs clinical record review and general chart audits per the PI Plan, federal and state regulations and guidelines.
  1. Generates and presents quality reports as requested.
  1. Works closely with all levels of Management and staff in regards to the tracking of sentinel and/or adverse events and translating this to the QI process.
  1. Acts as quality liaison for Masonicare Health Centers by participating in professional advisory committee meetings.
  1. Develops PI projects based on KPIs and facilitates implementation.
  1. Participates in affiliate and other requisite committee/board meetings and prepares educational sessions or materials for staff as required. 
  1. Participates in CMS and DPH audits/visits/investigations as required/requested.
  1. Performs other related duties as assigned.
  1. Creates QAPI Program and ensures monitoring of quality assurance projects in progress alongside all management staff, all departments.
  1. Prepares and coordinates survey readiness.
  1. Provides direction and coordination for the development and implementation of the quality improvement plan for all Departments.  Directs the program with the goal of ensuring that improvement occurs as a result of quality improvement activities.
  1. In conjunction with Department Managers, conducts Performance Improvement Projects at the facility using project-provided claims data using the CMS Quality Assurance Performance Improvement (QAPI) principles.
  1. In conjunction with Department Managers, works with facility leadership on preventative action plans based on QAPI audit findings, deviations, and events related to potentially avoidable transfers, plans follow-up audits/PDSA cycles to assess if the desired effect/change was achieved.
  1. Develops training materials and presentations for nursing facility staff based on areas of improvement defined in the Performance Improvement Projects. 
  1. Oversees IP programs, ensuring consistency with best practices and regulatory compliance. 
  1. Oversees and coordinates Education programs, ensuring consistency with best practices and regulatory compliance. 
  • Educates staff on quarterly outcome improvement efforts
  • Directs, Supervises, and Participates in the efforts to successfully orient new hires into work settings at Masonicare Health Centers.
  • Collaborates with the Staff Development Nurse to ensure areas of importance and high risk are being addressed to meet strategic initiatives. 

MINIMUM QUALIFICATIONS:

  1. Minimum of Associates Degree in Nursing from an NLN accredited college or university. Baccalaureate Degree preferred.
  2. Connecticut Registered Nurse license required.
  3. Five (5) years experience in acute care/home care/public health administration/SNF.
  4. Two (2) years experience in quality management preferred.

  1. Demonstrates strong organizational, written and verbal skills.
  2. Demonstrates ability to compile information, interpret data, and establish correction plans related to all quality management functions within the scope of position responsibility.
  3. Demonstrates knowledge of state, federal and accrediting body requirements and regulations.
