Day Shift / 40hrs/wk
This position provides oversight and support of training new Liaisons and coaching existing liaisons, as well as supporting the sales director with the day-to-day oversight of our CRM. This position supports census growth through strategy implementation and developing referral relationships.
Essential Duties and Responsibilities:
- Develop and maintain relationships with physicians, discharge planners, case managers, social workers, and other healthcare professionals that utilize homecare and hospice services.
- Be involved with recruiting and interviewing potential new team members.
- Assist Director of Care Transitions in management of CRM platform
- Collaborate with Director of Care Transitions to create yearly goals.
- Assist the Director with developing marketing plans that include sales and communication plans that will support census growth.
- Actively supports liaison team in census challenged areas and/or new projects, providing long-term and short-term support as needed.
- Conducts coaching sessions with liaisons including 1:1 meeting, ride along meetings, and sales meetings
- Supports liaisons to achieve increased continuum utilization of services.
- Ensures that the liaison team meets or exceeds individual admission goals on a weekly, monthly, quarterly, and annual basis utilizing organizational data and analytics, relevant market data and CRM system maximization.
- Completes liaison performance reviews in consultation with the Director. Monitor teams’ progress, identify shortcomings and propose improvements as needed.
- Implement the training program with new Liaisons.
- Responsible for providing a weekly update regarding marketing efforts, barriers/obstacles to reaching goals, etc.
- Responsible for communicating regarding the need for value added programming that will support census goals.
- Represents the Business Development team in the Director’s absence.
- Maintains knowledgebase of the Masonicare continuum of services.
- Other duties as assigned.
Minimum Qualifications:
• Must have valid and unencumbered CT. driver’s license
• Must have vehicle and be able to travel between accounts within Connecticut
• Must speak English. Bi-lingual preferred.
• Must have an understanding of DPH regulations
• Must have an understanding of Medicare, Medicaid & Managed Care Insurance
Experience: Bachelor’s Degree preferred. Prior experience in a Nurse Liaison or Community Liaison role is required.
