Day Shift / 16hrs/wk / EOW
Shift: 8:00am - 2:00pm
Under the direction and supervision of a licensed nurse, performs basic nursing care procedures. Implement a variety of recreation programs and services to residents that are socially, physically and cognitively stimulating.
Essential Duties and Responsibilities:
1. Performs basic nursing care procedures under the direction and supervision of a licensed nurse. Follow treatment plan that was designed to meet the appropriate age, needs and abilities of each resident/patient including assisting with meals and nourishment’s, transferring using proper body mechanics and appropriate assistive devices, ambulation, bathing, toileting and dressing and grooming.
2. Observes and communicates pertinent information about the adult/older adult resident/patient’s condition to the appropriate facility staff.
3. Takes and records pulse, respirations, blood pressures and weight as needed.
4. Documents care and observations on work sheets and EMR accurately and in a timely manner.
5. Maintains cooperative relationships to enhance the efficient operation of the care team by providing support and assistance to co-workers.
6. Adheres to infection control practices.
7. Supports the psychosocial needs of the residents as per the plan of care and profile.
8. Participates in educational and inservice meeting to update knowledge of techniques, guidelines and regulations. Must attend all mandatory programs. Meets basic requirement for 12 hours of continuing education per year.
9. Uses equipment in an appropriate and safe manner. Adheres to standards of the Diligent Minimal Lift Program.
10. Create and implement activity plans for residents, in the way of small groups, special events and entertainment. Motivates residents for participation in either planned groups or on a one-to-one basis.
11. Engages residents in activities to improve residents’ physical, social and emotional well-being
12. Enhances the quality of life of residents through engagement while promoting social interaction amongst residents
13. Observe and maintain safety of residents in falls program
14. Performs other duties as required.

Minimum Qualifications:
Education: High School diploma or GED preferred
Experience: 6 months – 1 year experience as a CNA preferred
Certificates, Licenses, Registrations:
State of Connecticut CNA Certification; or within
120 days of CNA Certification course completion.
