Summary of Position: Under the direction of the Director of Rehab Services, performs diagnostic assessments. Plans and implements specific treatment programs and establishes appropriate discharge plans for individual resident/patient participation in selected tasks to restore, reinforce and enhance performance; facilitates the learning of those skills and functions essential for adaptation and productivity. Understands the mission of the organization by recognizing the elderly we serve as unique persons requiring individual alternatives to maintain full lives in their later years by providing medical, spiritual and emotional care, encompassing quality, dignity, compassion and confidentiality for each individual.
Essential Duties and Responsibilities:
1. Assesses assigned residents/patients by evaluating data from a variety of sources; incorporates normative findings for older persons; utilizes appropriate tests and measures; identifies therapy diagnosis and prognosis and establishes realistic and measurable short and long term goals considering level of care and discharge destination. Able to assess residents/patients with a variety of diagnoses and in multiple practice settings. Adapts handling skills to meet the needs and abilities of older persons.
2. Establishes realistic individualized treatment programs for residents/patients with a variety of diagnoses and in multiple practice settings. Treatment plan is comprehensive and is geared toward improving functional outcomes.
3. Prepares legible, concise and organized written documentation (assessments and reassessments, treatment plans, progress notes, plan of care) that meets state and federal regulations and department guidelines. Written documentation supports billing. Is proficient in use of computer as it relates to documentation, billing, e-mail and medical record review and any other areas related to patient care or regulatory requirements.
4. Demonstrates adequate organizational and time management skills to complete job duties within required time frames and according to established schedules and workflow requirements. Meets productivity standards as established by the department.
5. Follows established departmental policies, procedures and objectives; quality assessment and improvement programs; and safety, environmental and infection control standards. Reports all unsafe conditions to appropriate supervisor including change in resident/patient status, broken or malfunctioning equipment or anything that could be a potential threat or danger to a resident/patient, staff member or visitor.
6. Maintains a harmonious working relationship with supervisor, development and colleagues. Demonstrates tact and diplomacy when interacting with residents/patients, staff families and other health care providers and vendors. Actively participates in interdisciplinary patient care planning, meetings/rounds, family meetings and departmental staff meetings.
7. Participates in professional development and continuing education on an ongoing basis including participation in educational programs, review of current literature, workshops and department/facility inservices with specific emphasis on enhancing knowledge related to working with adults and the geriatric population.
8. Demonstrates adequate skill in discharge planning including timeliness of discharge of services or progression to the next level of care in the continuum. Ensures post-hospitalization/treatment needs of the resident/patient are identified and arranged prior to discharge. Establishes and regularly upgrades home programs.
9. Maintains flexibility in work schedule and work assignment to meet the needs of the department as directed. Actively participates in department and facility committees and task forces. Serves as a clinical instructor for students.
10. Understands chemical(s) used in the department under the "Right to Know" standard.
11. Understands and follows "Universal precautions" as it pertains to the department.
12. Performs other duties as deemed appropriate by the Director of Rehab Services.
Education: Master’s Degree in Speech Language Pathology
Experience: Exposure to LTC and geriatrics
Licensure: Current CT licensure/certification as a speech language pathologist
Knowledge/Skills/Abilities: Clinical knowledge in multiple practice setting. Clinical knowledge of the geriatric population. Knowledge of federal and state regulations. Knowledge of financial and legal aspects of healthcare. Good interpersonal and oral/written communication. Self-motivation, able to work independently. Strong organizational and time management skills. Ability to research information. Knowledge of calculator, fax, copy machine and computer.
