Summary of Position: Under the direction of the Director, Social Services, provides assessment, services coordination, support and counseling to geriatric patients/residents and their families. Understands the mission of MASONICARE by recognizing the elderly we serve as unique persons requiring individual alternatives to maintain full lives in their later years by providing medical, spiritual and emotional care, encompassing quality, dignity, compassion and confidentiality for each individual.
Essential Duties and Responsibilities:
1. Visits patients on admission and conducts assessments which are age and culturally relevant and address the patient/residents social, emotional, physical and environmental status. Member of multidisciplinary patient care team.
2. Monitor plans and provides continuing contact with client/family to coordinate ongoing care. Acts as a liaison with nursing/medical staff and community agencies to facilitate problem solving and coordinate services. Acts as an educational resource.
3. Participates in program research activities and develops and implements studies related to client group and social work when assigned.
4. Provides counseling, social support, and assistance in crisis situations and when conflicts arise with service providers.
5. Prepares and maintains required documentation on each resident/patient.
6. Serves as discharge planner on assigned units. Introduces resources designed to meet the needs of the elderly after discharge and make appropriate referrals and arrangements for services.
7. Consults and keeps the Director informed of activities, needs and problems related to caseload. Has involvement in resolution of complaints, abuse allegations, and referrals to adult protective services.
8. Attends meetings as required. Enhances professional growth by attending seminars, workshops and inservices.
9. Understands and follows universal precautions and chemicals that are used in the department.
- Facilitates groups with residents/families as required per regulation (Resident Council, Family Forum) and develops additional groups PRN.
- Performs as resident/family advocate and assist team members to ensure realization of resident rights.
- Educates staff and residents as to bill of rights, including but not limited to grievance and abuse reporting.
- Maintains on-going communication with managed care insurance carriers for authorizations and notices of non-coverage.
- Completes Ascend documents for admissions to ACU and SNF and for level of care change notifications.
- Performs other duties as required.
Minimum Qualifications:
Education: Bachelor's Degree in Social Work
Experience: 1-3 years of experience in a medical or geriatric setting
preferred. 3-5 years experience required for positions covering the Acute Care Hospital Unit (ACU)
Certificates, Licenses, Registrations:
Certification Preferred
Key Competencies: Skilled in group process, counseling, verbal, written and organizational skills. Team building and computer skills.
