RN Coordinator/Designee (DAY SHIFT)

Summary of Position: The RN Coordinator/Designee is responsible for providing assistance with leadership, management and assessment skills for the ALSA Director 

 The RN Coordinator/Designee assists with managing the resident care services that are provided in the community. The management part of the RN Coordinator will include supervision of the RCAs and LPNs, resident assessments, and management, as defined by the ALSA Director.

Essential Duties and Responsibilities:

1. Performs initial and ongoing nursing assessments utilizing the nursing process to establish a comprehensive goal directed resident plan of services. Develops, implements and evaluates resident plan of services. Provides coordination of the plan of services and maintains continuity of care with other professionals.

2. Conducts supervisory visits of resident care assistants, companions and homemakers as required.

3. Implementation or delegation of responsibility for the availability of nursing services on a twenty-four (24) hour basis. On call responsibilities shared with SALSA. Flexibility to work open nursing shifts when needed to include all 3 shifts.

4. Medication management and physician order follow through. Responsible for medication administration and delegation of supervision of self-administered medications as specified in subdivision (4) of the subsection. (ALSA Regulation)

5. Transcribes, implements and evaluates physician orders in a timely manner. Documents accurate, pertinent and comprehensive information in a timely and concise manner

6.Nursing services, which shall include, but not necessarily be limited to:

Provides education, supervision and counseling to resident and family regarding Assisted Living procedures and other care needs.

Qualifications:

Education:                  A Baccalaureate Degree in Nursing with two years full time experience (one year in home health care or community health required) OR A diploma or Associate's Degree in nursing with four years of full time experience over the past 10 years (one year of home health care or community health required)

Licensure:                   Registered Nurse with valid Connecticut license

Knowledge/Skills/Abilities:

Must possess strong clinical, organizational, record keeping and documentation skills; able to function independently and exercise sound judgment and initiative; able to respond calmly to crisis or emergency and have strong facilitation and team work skills.  Position requires excellent oral, written and interpersonal skills.  Previous management experience and knowledge of State of Connecticut ALSA regulations and the Medicare and Medicaid programs is required

Working Conditions:  Exposure to body fluids, infections, odors and varying resident behaviors should be expected.   Position may require long irregular hours and may be emotional and/or stressful due to issues and/or crisis affecting the health and well being of the residents.
