RN Homecare Clinical Manager
Job Location: Glastonbury, Connecticut Apply Via ApplicationDay Shift / 40hrs/wk
This position is accountable for ongoing planning, organization, direction, evaluation and safe delivery of high-quality care to clients in accordance with agency Policies and Procedures, State and Federal Regulations. The Clinical Manager for Clinical Services provides oversight of all patient care services and personnel.
The role reports to the Director, Clinical Services. This role supervises the RNs and LPNs in the field.
Essential Duties and Responsibilities:
- Accepts and coordinates appropriate referrals to the agency.
- Oversees all patient care services and personnel.
- Oversight must include the following:
- Making patient and personnel assignments,
- Coordinating patient care,
- Coordinating referrals
- Assuring that patient needs are continually assessed, and
- Assuring the development, implementation, and updates of the individualized plan of care
- Responsible for the supervision, instruction, support and coaching of field staff in providing quality homecare services in an organized, cost efficient manner.
- Accountable for the management and supervision of field staff in the areas of time management and productivity.
- Coordinates inter-disciplinary team and individual case conferences.
- Utilizes decision support tools to monitor branch, identify trends, plan and implement; and prepare variance reports for Regional Director of Home Health and/or Corporate Director of Rehab and/or Hospice Director.
- Participates in Administrative on-call on a rotating basis
- Evaluates the performance of assigned staff in meeting productivity, quality standards and documentation standards
- Conducts in home supervisory visits with assigned staff.
- Coordinates efforts with other members of the management team to ensure ongoing development and improvement of PSA and organization wide activities
- Maintains an open line of communication with all levels of staff
- Directs individual subordinates and groups to appropriate resources to resolve problems
- Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.
- Can be depended upon to report to work on-time, use time off appropriately, and complete annual education and medical requirements.
- In conjunction with Human Resources, participates in staff recruitment, selection and discipline/termination of employment of staff not performing according to agency standards
- Identifies, develops and provides in-service/continuing education for the staff
- The Clinical Manager’s core responsibilities will include, but are not limited to:
- Coordination and management of all services rendered to patients and families by direct service staff under his/her supervision.
- Supervision of assigned personnel in the delivery of services to patients and families
- Direct evaluation of the clinical competence of assigned personnel and participation with appropriate supervisory staff in the evaluation of other direct service staff
- Participation in or development of all agency objectives, standards of care, policies and procedures affecting clinical services
- Participation in direct services staff recruitment, selection, orientation, and inservice education
- Participation in program planning, budgeting and evaluation activities related to the clinical services of the agency.
- Performs related and other duties as required.
Minimum Qualifications:
Education: BSN preferred.
Experience: Demonstrated leadership ability and commitment to the provision of quality Care. Knowledge of State Regulations, Medicare and Medicaid Guidelines is desirable.
Certificates, Licenses, Registrations: Current Connecticut Registered Nurse License. Maintains current Driver’s license and auto Insurance
