Summary of Position: The Hospice Licensed Practical Nurse (LPN) is responsible for and provides skilled nursing care reflective of the agency's mission to the ill or injured in the home, hospital or extended care facility. The LPN works under the direction of the Nurse Case Manager and/or Clinical Supervisor.
Essential Duties and Responsibilities:
Responsible for coordination of care with community and agency resources and with the designated registered nurse on a regular basis.
Provides nursing care as per the established plan of care and documents clinical activities per the agency policy.
Adult and geriatric client assignments and reports are received from a registered nurse/designee. Performs procedures and treatments accurately and documents appropriately.
Observations of adultand geriatric client's condition are accurately reported to the designated registered nurse.
Communicates adultand geriatric client care issues or concerns to the designated registered nurse or Clinical Supervisor in a timely manner.
Assists adult and geriatric clients in meeting socialization needs. Promotes education for adult and geriatric clients, client families and the community.
Documents timely and accurate computer entry of patientjob information to facilitate the development of orders for service, creation of time sheets and other required patient/employment documents and reports.
Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.
Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements.
Performs other duties as assigned by Hospice Supervisor of Clinical Services/designee.
Education: Graduate of a NLN accredited school of nursing required.
Experience: Experience in home health care or hospice preferred.
Certificates, Licenses, Registrations: LPN license and Current drivers’ license and auto insurance.

Demonstrates understanding of nursing process, interpersonal relationships and dynamics. Maintains positive professional demeanor. Demonstrates organizational, written and verbal skills. Demonstrates basic PC skills. Attends 12 hours of in-services per year, 6 of which are hospice related. Provides care and services that demonstrate behaviors and the principles of person-centered care including affording people dignity, respect and compassion, offering coordinated care, support and treatment and care that is personalized.
