Full Time - Day Shift

Summary of Position:     

Under the direction of the MBHH Director, Social Services, provides group, individual and family therapy to geriatric population.  

Essential Duties and Responsibilities:

  1. Completes psychosocial assessment, Plan of Action, and Plan of Action updates within established timeframe.
  2. Initiate (upon admission) discharge planning in collaboration with patient and treatment team.
  3. Demonstrate skill of writing individualized, behavioral, and measurable treatment goals based upon information from psychosocial assessment and psychiatric evaluation.
  4. Conduct daily group therapy sessions.
  5. Conduct individual and/or family therapy sessions as defined by treatment plan.
  6. Collaborate with psychiatrist to ensure coordination of care.
  7. Schedule medication management appointments.
  8. Obtain insurance benefit information and authorization for treatment; perform ongoing utilization reviews.
  9. Demonstrate skill in documentation to meet payer and regulatory standards.
  10. File all progress notes in the medical record within 24 hours of Date of Service.
  11. Respond appropriately to patient’s requests.
  12. Provide comprehensive education, instructions, and/or advocacy information to assist patients and significant others to secure necessary health care follow-up services.
  13. Provide referring providers and/or therapists with biweekly updates of patients. compliance with and progress in treatment.
  14. Adhere to policies and procedures.
  15. Prepare and assist with audits when requested.
  16. Assist with community and marketing events, as needed.
  17. Demonstrate ability to work and communicate effectively as a team member with providers and other staff.
  18. Demonstrate ability to organize and prioritize tasks effectively.
  19. Performs other duties as assigned. 

Minimum Qualifications:

Education:  

Master's Degree in Social Work or related field

Experience: 

2 years of experience in a behavioral health  or IOP setting preferred.

 Certificates, Licenses, Registrations:

LCSW. LPC, LMFT
