Masonicare Home Health & Hospice, an affiliate of Masonicare, is seeking a dedicated and accomplished OCCUPATIONAL THERAPIST (OT) for our EAST LYME, CT service area.

This is a PAY-PER-VISIT OPPORTUNITY.
The Occupational Therapist (OT) provides therapeutic services according to physician's orders in the patient's home setting.
The OT will perform patient evaluations and will work to develop, implement and evaluate the plan of care and make necessary revisions to address all problems in the plan of care. OTs provide coordination of the plan of care, maintaining continuity of client care with other health professionals while identifying interdisciplinary care needs and making recommendations for additional home health care services.
Masonicare is fully electronic! Our therapists demonstrate timely and accurate computer documentation of patient/job information to facilitate the development of the OASIS data set, orders for service, creation of time sheets and other required patient/employment documents and reports.
Visits are done primarilly during the DAY SHIFT, but exact scheduling is flexible.

Minimum Requirements
Current Occupational Therapy License and one (1) year of clinical experience of occupational therapy in a progressive therapy environment required.
